Your club may request documents from you — medical forms, consent forms, insurance certificates. Upload them here.
Viewing requests
Go to Documents to see all document requests from your club. Each request shows what's needed, the due date, and its current status.
Uploading a document
- Click on the document request
- Upload a photo or PDF of the document
- Submit for review
The club admin reviews your submission and either approves it or asks you to resubmit. You'll receive a notification of the outcome.
Document statuses
- Pending — You haven't uploaded yet
- Submitted — Uploaded and waiting for review
- Approved — Accepted by the club
- Rejected — Not accepted, please resubmit
- Expired — Needs renewal
